Error Message - Help please!

I keep getting this error message when I try to transfer my Access db to MySQL -

Why is this? I have the right MySQL version, the right ODBC and the right Access to MySQL. I don't see a spot to put the name of the exact MySQL database to transfer to. How can I get around this?????
